Check your eligibility
You are at least 18 years old on the date of your first appointment
Did you complete a new patient exam/dental service, including but not limited to cleaning, consultation and x rays.
This is your first and only time claiming a reward with Dentiva and the reward is for the person name registered.
That you registered with the CDCP and provided your application number to Dentiva
You completed the online review on the Dentiva website and provided all applicable information
You fully understand that Dentiva is supporting Canadians in their registration for CDCP and finding suitable dental care.
